2. Show proof of your current address (if not listed on ID)
Voter registration
Car registration
Utility bill, phone bill, rent, or mortgage receipt
Mail addressed to patron at current address
Printed checks with current address
Any other legitimate legal document showing name and address
3. Bring your parent or guardian with you (for minors under age 18 only)
For minors (under age 18), the parent or guardian must complete and sign an application form on behalf of the minor. The parent/guardian AND the minor must be present when the card is issued.
If parent/legal guardian has a valid CPL card, this can be used as verification of address and parent/legal guardian does not need to show additional ID.
If parent/legal guardian does not have a valid CPL card, a photo ID and verification of address must be shown
4. Accept personal responsibility for outcomes resulting from the use of your card
As a holder of a library card, it is your responsibility to:
Be financially responsible for items borrowed with this card.
Keep your account up to date with correct contact information.
Monitor the items your child selects and checks out on this card.
Report a lost or stolen card immediately. (You are solely responsible for items borrowed on your card until you report it lost or stolen.)
